Can a Family Member Apply for a Birth Certificate on My Behalf?

Requirements for a Family Member to Obtain the
Computerized Birth Certificate

  • Notarized Letter of Authorization
  • copy of applicant’s identification (must be valid)
  • copy of present birth certificate
  • Persons applying on behalf of someone must also produce their valid identification.
